Infection Control Policies*

1. If you or your child(ren) are symptomatic, please alert the office prior to your visit.

2. Only 1 parent will be permitted to attend visits with their child. Parent must not have any sick symptoms. All other family members, including siblings, will not be permitted inside the office.

3. All adults and children over 2 years of age must wear a mask when entering the office AND throughout the entire visit.

4. Please complete New Patient Paperwork prior to your visit to reduce exposure.​

5. Please alert the office if you, as the parent/guardian, have a known infectious disease such as Covid, Flu, etc.
